Responses to the following questions may be up to 500 words in length and should address all components of the prompt.

Please describe a specific instance where you demonstrated leadership skills. How did you handle the situation, and what was the outcome? Reflect on how this experience has prepared you for a future role in healthcare.

Discuss a volunteer or community service experience that has had a significant impact on you. What were your responsibilities, and how did this experience shape your understanding of community engagement and service? Please include the duration and level of your involvement.

Share a detailed account of your dental shadowing experience. What specific areas of these experiences were most enlightening, and how have they influenced your decision to pursue a career in dentistry? Include the duration and key learnings from your shadowing activities.

Describe your involvement in extracurricular activities. What have you learned from these experiences, and how do they contribute to your personal and professional development? These activities do not need to be related to health or dentistry.
